Gaming & Esports
Sign-ups for squads, ladders, and LAN nights.
Not a screenshot of a form — a real one. Register a squad below, switch it between guided and page mode, feel the whole thing. This is the actual FormNest tournament template, running live.

Why FormNest for gaming
Built for gamers, by builders who get it.
We build forms for a living, so we know a tournament sign-up is not a contact form with a new coat of paint. What’s above is live right now — and we’d rather show you exactly what it does today than promise a highlight reel. Straight up: the gamer tag, platform, rank, and Discord questions are captured with standard text and dropdown fields for now. They do the job; they are not yet purpose-built types. Here’s what genuinely ships today.
Two modes, one schema
The same registration ships as a classic page or a guided, one-question-at-a-time flow — like a pre-game lobby check-in. Flip the mode above; it is a setting, not a rebuild.
Logic that branches
Conditional show/hide means a solo entry never gets asked squad questions, and a console player never sees PC-only fields. One form adapts to each entrant.
Yours to brand
Real control over how the form looks — colour, background, the whole feel — so a LAN night's sign-up looks like the event, not a generic grey box.
On the workbench
Purpose-built for the scene — soon.
This part is the plan, not the product. Today’s form captures everything a tournament needs with standard fields; the next phase turns the gamer-specific ones into field types that understand what they hold.
- Gamer tagA dedicated gamer tag field will become a purpose-built handle field — format-aware, with the platform it belongs to attached — instead of the plain text box capturing it today.
- Rank / tierA dedicated rank / tier field will ship as a native tier selector with the ladder built in, so seeding and brackets can read rank directly rather than from a free dropdown.
- PlatformA dedicated platform field will grow into a first-class platform picker that downstream logic understands, driving cross-play rules without hand-wiring every condition.
- Discord handleA dedicated discord handle field will validate as its own field type rather than plain text, so a roster arrives with every handle in a consistent, checkable shape instead of needing a tidy-up pass before check-in.
Every line above is future tense on purpose: none of these purpose-built types is live yet. When one ships, it moves up to the live form — and this list gets shorter.
